When you become a certified agency and received your PTAN, you will need to discharge and readmit your patients that are on service as of your certification date.
On the Hospice Tools Intake page, you will select 'Discharge for Cause' as the reason for discharge.
The patient was only discharged due to becoming certified and now the new chart is what is reported to Medicare.
Selecting Discharge puts a billing code of 01 on the final claim to reflect the end of care, which is what the claim needs, but for this situation there is no discharge claim set anyway.
