To use the Medicare Beneficiary Identifier (MBI) Lookup tool in eServices, first log in to the eServices portal. Then, navigate to the MBI LOOKUP tab and enter the required fields: the beneficiary's last name, first name, date of birth, and Social Security Number (in the XXX-XX-XXXX format).
Select the "I'M NOT A ROBOT" checkbox and click SUBMIT INQUIRY to see the result.
